For HPT Students Ready to Graduate


Steps to follow:

1. During the second to your last semester before you graduate, you want to check the Schedule of Classes to uncover the DEADLINE to submit your Graduate Application.

 

2. To apply for graduation, which you do online at the GSU website, follow these attached steps. Click here for instructions.

 

Right after you apply online submit these completed forms to Dr. Lanigan

FORMS needed - complete and e-mail to Dr. Lanigan:

Click here to print HP&T Exception form - 50% or more credit hours with one faculty

 

Here is another form you may need:

If took PowerPoint I and/or II with Professor Thomas as a substitute for HPT 5100 (old HPT 522), OR, if you took Education 7212, then you need to fill out a course substitution form. Click here to download the form. Complete the form by filling in your name, college (i.e., CAS), major (i.e., HPT), and student id. Next, go to the section called Substitutions. If you took Coms5050-PowerPoint I, then under the Required field, write: HPT 5100/1 (the 1 stands for one credit hour). On the next line, under the Sbustituted field, write: COMS5050/1/* (the * should be substituted with the grade you received) . If you also took PowerPoint II, then on the next line under the Required field, write: HPT 5100/1. On the next line, under the Sbustituted field, write: COMS5050/1/*.

If you took Educ 7212, then on the Required field, write: HPT 7250/3 (the 3 stands for three credit hours). On the next line, under the Sbustituted field, write: EDUC 7212/3/* (the * should be substituted with the grade you received) .

Sign the form. Include this form with the other required forms that go to your advisor.

Note: The original form provided by the Dean's office is slanted. Thus, if you print out the form, it will look crooked. Don't worry about it.
